What is the purpose of the pre-screening form?

The purpose of this form is to START the conversation about Vivos with your patient and the GOAL is to schedule them for a Patient Education Meeting. This is the “planting the seed” moment referenced in Session 3 training.

At what point in the patient flow do you incorporate this?

First step. This is the biggest way to raise awareness within a practice’s general dentistry patient base; begin with introducing this form to EVERY patient.

How to Screen Patients In-Office:

All New Patients are to be given a Sleep and Breathing Questionnaire, along with the other New Patient paperwork they are asked to complete.

All Existing Patients are to be given a Sleep and Breathing Questionnaire as part of their health history update when they are in the practice for their re-care or other appointments. (If this is the patient’s first visit since implementing Vivos into the practice.)

PUTTING THIS INTO ACTION

SCENARIO: How does this look once implemented?

The patient arrives into practice — front office opens with “Great, we have you all you checked in. [Doctor’s name] is incorporating more on breathing and sleep wellness. This is the new standard of care in dentistry. He/she is really wanting to see how you’re doing. We are still getting your room ready. Go ahead, fill this out and we’ll get you in the back.”

Once in the back, doc/hygienists already know the physical signs to look for — but how are they building the bridge to those physical signs to the patient’s symptoms? The symptoms are what helps bring about that awareness.

“You may be wondering why I asked you to fill this out, I’m noticing some red flags — tell me more about hypertension, waking up feeling tired.” Use the information patient has filled out on the Sleep Breathing and Health Questionnaire.

Pause and wait for their response.

“What if I told you that the many things you just mentioned were all linked back to ONE possible underlying cause and that’s something we are now treating in our practice. We host group education meetings that I think you would really benefit from attending. Our next workshop/meeting is….. “
